My Account and some online forms may be unavailable for short periods on Tuesday 5 July while we carry out routine maintenance work. We apologise for any inconvenience caused.

You can get in touch to find out what support you may be able to access either directly from adult social care or through one of our partners.  

To ensure your needs can be appropriately met, the Adult Social Care team will need your consent to share your personal details with other agencies.

Ask for support from adult social care to request an assessment or report a safeguarding concern 

Assesments

If your referral is to request an assessment, it will be reviewed and triaged by a Customer Service Officer. 

They will work with you to find out what services will best help you to live a more independent life.  

This may include referring you to other services which are not directly provided by adult social care.

We may need to share information about you with other agencies that become involved with your care such as the Police and Fire Service, or others such as health professionals or the voluntary sector.  

We will ask you to provide consent to contact other agencies as part of the referral.

We aim to complete assessments within 28 days.

In an emergency we aim to provide help and support within 24 hours.  

If your case is not urgent, we expect to respond to your referral within 5 working days.  

However, these timescales can sometimes be subject to change.

If you are in hospital, a social worker will discuss the help you and your carer will need when your discharge from hospital is being planned.

Safeguarding

If you’re reporting a safeguarding concern, your referral will be reviewed by a social worker.

They will work with you to find out more about the concern what help maybe be available to support your wellbeing and safety

This may include referring you to other services which are not directly provided by adult social care.

We will ask you to provide consent to contact other agencies as part of the referral. 

We aim to respond to safeguarding concerns within 24 hours.

If your case is not urgent, we expect to respond within 5 working days.

However, these timescales can sometimes be subject to change.

Charges

You may be charged for the services you receive.  

We will carry out a financial assessment to work out if you need to contribute to the cost of your care.

As part of the assessment, we will also make sure you are receiving all the benefits that you are entitled to.
