Referral details

Information required within the referral form include the needs of the child / young person, conditions, why a referral is being made, agreed short breaks package, behavioural information and sensory needs. Once we have agreed to complete an assessment from the information on the referral form and discussions with the family / social worker we will begin to complete an impact assessment. Description

Meridian is one of the Together Trust’s short breaks services. We are a leading disability charity in the North West. 

Meridian supports children and young people up to the age of 18 with autism and additional / associated needs.

Meridian enables the whole family to benefit from the service by providing short breaks for children and young people. We support children and young people to access activities, have new and fun experiences, make links with the local community, form friendships and build positive relationships. Families feel secure that their child is in a safe and stimulating environment with a trained and caring staff team who can meet their needs. Meridian also supports children and young people with life skills, including communication and independence.  We’ve put a lot of love, care and attention into designing our home to create warm, friendly and nurturing spaces. We support and encourage the children and young people to thrive, get involved and feel comfortable. It really is a home from home. 

We’ve got a sensory room, low stimulus spaces and an outdoor play area. Not to mention our lovely communal spaces. With space for up to five children each night, we offer personalised care and plenty of opportunities for your child to learn, socialise and play.
