If you are asked to add a service that you already have access to, please do not try to add the service or request a PIN. We hope to resolve the issue soon and apologise for any inconvenience caused.

3. Supplementary form

A supplementary form should be submitted alongside your application. The simplest way of doing this is to upload the form during the application process.

Failure to return this form will cause delay to your application.

We aim to process all fully completed applications (in most cases this requires a fully completed supplementary form) within 15 school days.

We will then email you to confirm any offers or next steps. Any delay in us securing a supplementary form may delay the processing time of any application.

If it isn’t possible to have the form completed before you make your application, you can send it to the school admissions team by email or post after the application has been made. Details of where to send it to can be found at the top of the form.
