After completing our COTT referral form, you will be invited to attend the clinic, where possible, at The Link Centre.
Only once the assessment has been completed, can your needs be met either through the provision of information and advice, appropriate equipment, minor or major adaptations in line with the Disabled Facilities Grant (DFG) legislation or signposting to other services.

The Community Occupational Therapy Team does not provide wheelchairs, please request a referral to Wheelchair Services via your GP. Alternatively, you may be able to rent or buy wheelchairs from suitable suppliers.
If your referral is requesting support with walking and overall mobility, including the risk of falls, please discuss a referral to Physiotherapy with your GP.
