Prevent referral form

By sending this form you acknowledge that it will be sent to both your dedicated Local Authority safeguarding team and Prevent policing team for a joint assessment. 

Wherever possible we aim to give you feedback on your referral, please be aware, however, that this is not always possible due to data-protection and other case sensitivities.
