Referral detailsMany of our placements are driven by families who recognise the uniqueness of our services. Following an initial visit, the starting point for any placement is Social Services or,when relevant, the Health Authority. They, in turn, make a referral which is a request for an assessment. Our comprehensive needs led assessment ensures we understand the individual's needs and aspirations. The process involves meeting with family, school and any other stakeholders in the individual's life. Where we can meet an individual's needs, the appropriate home is identified.
Venue notesOur homes are all based in Lincolnshire however the majority of our residents placed with us come from outside of our local area, as the provision they require is not always available to them in their own area – we currently work with 37 Local Authorities nationwide.

Day Opportunities supports the delivery of a number of workshops from gardening to photography.
There are also regular activities including drama, music, arts and crafts, football, gym and swimming sessions.
