Sharing information and good practice for disabled young people, families & professionals.

We are a specialist network of the Council for Disabled Children set up to provide targeted information and resources about transition through online resources, publications and events.
We publish a magazine called My Future Choices which is for disabled young people, families and professionals. It includes articles written by disabled young people about their experiences of transition, information about transition projects, the latest policy and charity news and resources.

We work in partnership with the voluntary and community sector and with young people to develop training for professionals such as the: It's My Life events on the Mental Capacity Act and training on the Children and Families Act and Care Act.

We also work behind the scenes on policy issues, respond to government consultations and work with leading organisations to develop and promote good practice in transition.

Please be aware that we do not have a telephone helpline and we are not resourced to provide direct advice to disabled young people and their families. However, there are some amazing organisations out there who can help.
