Description

The Community Physiotherapy Service for adults provides assessment and care to people with therapy needs.

Members of the team (Physiotherapists and Assistants) visit people in:

  • Homes
  • Residential
  • Nursing home facilities
  • Day centres

We accept referrals to the service from GPs and other health care professionals.The team has strong links with the other community and specialist teams e.g. Community Neuro Rehabilitation Team (CNRT), Stroke team and Macmillan services.

The team's aim is to restore optimal function and promote independence and well being of patients in the community.

The emphasis is on guiding and facilitating patients to self manage their condition.
