Description

The Oldham Memory Service is run by Pennine Care NHS Foundation Trust.

People are referred by their GP. The service provides assessment and timely diagnosis of people with memory problems in Oldham.

It provides ongoing support and information to enable those diagnosed with dementia and their carers to live well with the condition.

The service also provides advice and education to patients, carers, GPs, to other health and social care professionals.
