Small Business Rate Relief Application Form

Section 43 (4D) of the Local Government Finance Act 1988 makes it a criminal offence to give false information when making an application for Small Business Rate Relief.  You must inform the council immediately of any changes in circumstances which may affect your entitlement to relief.
