Services for Children with Disabilities and/or Additional Health Needs
Who we are
Multi-agency team made up of staff from the Oldham Primary Care Trust,
Pennine Care and the Children, Young People and Families Directorate.
What we do
- Work with children and young people aged 0-18 years with severe learning
disabilities and/or complex/acute health needs in partnership with their parents and carers.
- Provide individualised packages of support based on assessed need.
- Promote and support the inclusion of children and young people with disabilities into
local community activities.
- Promote a co-ordinated approach by working with all agencies involved.
- Provide support around: assessment and diagnosis, play and development, health issues,
behavioural interventions, skill teaching, family-based and residential short breaks, survival skills
for parents, opportunities for leisure, access to benefits and financial support, transition to Adult
Services.
Availability
The office is staffed Monday to Friday 9am –5pm.

General enquiries to the team are welcome.
Referrals may be made by anyone with evidence of parental permission
unless there are concerns about the welfare of the child.
