Physical Disability Team

What the service is

The team provides assessment and care management for adults with a physical disability under 65 years of age.  They work in partnership with other statutory, voluntary and private agencies to enable  disabled people to be as independent as possible by supporting them and their families

Support they provide includes:

  • Submission to panel of “in control” documentation to enable calculation of indicative amount for individualized budgets.

  • Support with devising care plans and support plans and the implementation of individualized budgets and/or traditional care packages.

  • Social work supportI

  • Investigation of alerts of adult abuse to oldham residents who are eligible for a service from the team.
