The Community Independence Team is a domiciliary home support service with an overall aim to provide support to meet the individual needs of older people. Enabling them to live in their own home for as long as is practicable to do so.
This is achieved by providing:
Assistance or support as required for peoples day to day personal care and daily living needs
Support and encouragement to assist people to retain or develop skills to enable more independent living.
Support for people to access opportunities within their local community.
The service is aimed at older people living in the borough who have dementia, a functional mental health problem, or who have re-enablement needs.
This service can be accessed through the Adult Duty and Intervention Service. You will be assessed and if eligble, referred to this service or other services as appropriate:
