The Toolkit provides copies of forms on which schools can record interventions made in line with the recommendations of the SEN Code of Practice.

Services for Children with Disabilities and/or Additional Health Needs
The team works with 0 to 18 year olds with severe learning disabilities and/or complex/acute health needs and their parents and carers.
Referrals may be made by anyone with evidence of parental permission (unless there are concerns about the welfare of the child, when no permission is required).
