Needs assessments

Needs assessments and financial assessments help establish how much your contribution will be to care services you use.

Someone from the Council will assess your needs with you to see what services could help, then agree a Care Plan and work out how much money is needed to cover the cost of your care.

An assessment involves you and a trained member of our staff (you can also involve someone who looks after you, or anyone else).

The assessment can also include other people, such as your GP.

The information you give is confidential and will only be shared with people who need to know about your situation to help you. You will be asked for permission before information is shared.

Going into hospital

Hospital patients will automatically receive a needs assessment, you do not have to arrange one.
