What the service is

The service provides domiciliary support and resettlement services to support disabled people with an assessed need to live independently in their own homes  and in their local community, by providing person centred, health and social care re-enablement services.  The service provides a seven day boroughwide domiciliary service operating over a  24 hour period  including bank holidays. A range of services to adults with different support needs are provided across the borough, where re-enablement isn’t feasible because of the nature of the physical impairment or medical condition, a well trained staff team will maintain where possible existing levels of function, ensuring the people we support can remain at home.The Re-enablement Team works closely with the Borough Wide Response Team, working with service users across all ages.

The service supports physical disabled adults under 65.
